Important Information
At Collingwood Medical, we set our consultation fees based on the duration of your appointment, the specific services provided, and the complexity of your visit. Please note that additional charges apply for medical consumables, including non-government-funded vaccines and procedural equipment. The fee schedule serves as a general guideline.
As a private billing practice, Collingwood Medical enables patients with a valid Medicare card to receive a rebate for part of the consultation fee. We recognise that financial situations can vary and encourage patients facing financial hardship to speak with their physician. We are committed to finding affordable options, including discounted rates when possible.
We accept payments by card only, including all major credit and debit cards, with a merchant surcharge of 1.2% applicable to all transactions. If necessary, you can arrange to settle your account on the same day via bank transfer; please consult with us beforehand. All fees incurred during your appointment are due on the day of your visit. Late payments will incur a fee of $16 per month until the outstanding balance is settled.
For Telehealth appointments, payments are conveniently processed through HotDoc, which charges a fee of 1.75% plus $0.30 per transaction. Your card will be pre-authorised for the expected appointment amount at the time of booking, with charges applied only after the appointment concludes. HotDoc will hold the pre-authorised amount for 7 days if the appointment does not occur. When released, the funds may be reflected in your account in 3 to 5 business days.
To facilitate the transfer of your medical records to a new practice, please complete a Transfer of Medical Record Request form at your new practice. The completed form must be submitted to Collingwood Medical for processing. While we make every effort to process requests efficiently, please be advised that some transfers may take up to 21 days. We assure you that your confidentiality and privacy will be maintained throughout this process, and please note that a $45.00 fee is applicable for the transfer of medical records.
To uphold equity for all patients, we request a minimum of 2 hours’ notice for appointment cancellations. Should you need to cancel within this time frame or be unable to attend, a fee of $50 will be assessed.
Collingwood Medical welcomes foreign visitors and workers. Individuals from countries with a Reciprocal Health Care Agreement (RHCA) are eligible to access essential services. For those without RHCA coverage, it is mandatory to possess private health insurance. International students are reminded to maintain Overseas Student Health Cover (OSHC). For patients without valid Medicare, full payment is required on the day of the appointment, and a tax invoice will be issued for potential insurance claims.
We look forward to seeing you and discussing your individual needs. Give us a call or book online.
